Work around a doxygen misfeature: we want to refer to handles by their typedef names, not the struct tag. There is a doxygen setting that does part of what we want (TYPEDEF_HIDES_STRUCT), but it doesn't pick up references to types in ordinary text. We only have a handful of types: hard-code the conversions as macros in the doxygen config for now.
